KHSAA Physical Form

ALL STUDENT-ATHLETES MUST HAVE A CURRENT PHYSICAL FORM ON FILE BEFORE PARTICIPATING IN ANY TRYOUT, PRACTICE, OR TEAM ACTIVITY.

It's preferable that forms are submitted directly to Mercy's certified athletic trainer, Stephanie Uhrich, via hard copy or electronically (suhrich@mercyjaguars.com) ahead of any team activities. That way she is able to check that everything is completely and correctly filled out. If that is not possible, a coach may collect them on the first day your daughter participates with the team.


Mercy Academy Athletic Handbook

Mercy Athletics is home to 16 athletic programs, 26 teams, over 60 coaches, and over 300 athletes. Our student-athletes compete in accordance with KHSAA guidelines and represent Mercy with pride, commitment, and integrity.

Participation in athletics at Mercy is a privilege that comes with high expectations both on and off the field. This handbook outlines the policies, procedures, and responsibilities associated with team membership, as well as the values that guide our culture.

All athletes and families are expected to read and understand the contents of this handbook before the start of each season. Questions may be directed to your coach or the Athletic Department.

Mercy Athletics Participation Waiver

Participation in Mercy Academy Athletics is a privilege. All student-athletes and their parents/guardians are required to read and acknowledge the guidelines, expectations, and responsibilities outlined in this handbook.

Please sign and return to your coach or directly to the athletic office.

  • Hard or electronic copy is fine
  • One per student-athlete (no need to fill out multiple for multi-sport athletes).
  • Waiver is mentioned at the parent meeting and then again to rostered athletes.

Any questions can be directed to the athletic director.

Mercy Athletics Transportation Waiver

Mercy Academy Athletics utilizes various modes of transportation for practices, games, and other team-related events. These may include school buses, vehicles driven by coaches, or, in some cases, transportation arranged privately between students and their families.

Please sign and return to your coach or directly to the athletic office.

Mercy Academy Athletics Transportation Waiver 2025-26

Each student-athlete should have these waivers on file with the Athletic Office. We recommend turning both waivers in together and we are happy to collect them with your sports physical.


Emergency Action Plan

Mercy Academy has a written emergency action plan that should be activated and followed in the event of a medical emergency. All coaches should be familiar with this document and their role and responsibility in an emergency situation. Emergency situations are rehearsed with each team at the beginning of their seasons to prepare coaches, athletes, and the athletic staff. Any questions should be directed to the athletic trainer or athletic director.

KHSAA Dead Period

| June 25 through July 9 – this dead period is state mandated and applies to ALL high school athletes. During this time, high school athletes are not allowed to be involved with their high school teams or use their facilities. We encourage student-athletes and their families to take their family vacations during this time.
